General rates for the Standard Health Insurance Contract plus plan are as follows:
Currency in USD.
- Lifetime maximum for each individual under plan: $1,250,000
- Calendar year Maximum for individuals: $125,000
- Calendar year out of pocket maximum for individuals: $1,200
Dental
If you are have a dental policy with us, then your yearly maximum (for each individual under plan) is $2,000 USD per Calendar year.
Vision
If you are have a vision policy with us, then your yearly maximum (for each individual under plan) is $400 USD per Calendar year.

- Lifetime maximum for each individual 65 years and Older: $1,219,00
- Annual maximum for each individual 65 years and Older: $250,000
- Out-of-pocket annual maximum for on island services:

- Family: $2,000
- Annual deductible for off island and out of network services:
- Individual: $500
- Family: $ 1,000
